Chennai,  October, 2017:Dr. Agarwal’s Eye Hospital, Chennaitoday announced that they hadconducted for theFirst time in the World a successful pediatric, uniquesurgery for Monocular Elevation deficiency (MED) with congenital ptosis, adisorder of eyeswith congenital eye muscle problem. The surgery was conducted on a 4-year-old child with a complaint of drooping upper eyelid along with downward displacement of the right eye due to absence of elevation and therefore the child had vision impairment due to lazy eye since birth.

On thorough evaluation, thechild had severe vision impairment in the right eye due to congenital ptosis with double elevator palsy condition. The prevalence of sucha condition is 0.5% of all squint cases.

agarwal 2Speaking on the occasion Dr.ManjulaJayakumar, Paediatric Ophthalmologist, Dr. Agarwal Eye Hospital said, “The usual surgical practice is inferior muscle weakening done during the first sitting, followed by Knapp’s surgery done around 4 months later where the two horizontal muscles are transposed to the weak superior muscle to give elevation power. Instead of 2 sittings procedure with a treatment gap of 4 months, this unique procedure was done in a single sitting to the patient. The inferior muscle weakening along with only one muscle transposition to the weakened superior muscle with augmentation for improvement in elevation resulted in a good surgical outcome. The advantage of this procedure is that the child has a quick remedial action as a one surgical procedure for the squint thereby the ptosis (drooping eyelid) can be there after corrected in three weeks resulting in quicker visual rehabilitation of the patient as opposed to the conventional surgery which would take at least a five month period for the complete surgical recovery. Our patient not only had desirable cosmesis but also improved in vision from 6/60 to 6/18.”

Dr. Ashvin Agarwal, Executive Director, Dr. Agarwal Group of Eye Hospitals said, “We took it as a challenge and successfully performed this unique surgery and now we are happy to announce that this unique surgical procedure is being reviewed by JAAPOS, the journal of American Association for Pediatric Ophthalmology and Strabismus, as a new surgical procedure for Monocular elevation deficiency”.

Speaking on the occasion, Mother of 4yr old child said, “We were highly depressed on refusal by many hospitals to cure my child’s vision problem. With surgery done at Dr. Agarwal’s Eye Hospital, our child was able to get proper vision and the complaint of drooping eyelid and squint has been completely cured.”

This alternative surgical approach performed on the child yielded successful surgical results with one surgery to correct the entire strabismus followed by surgical correction for the drooping lid resulting in faster visual rehabilitation and reducing the number of surgeries and risk of general anesthesia in a small child.
